Your denial letter

The letter that says your plan will not pay. It may be called a "notice of denial" or an "adverse benefit determination".

Or your EOB

The Explanation of Benefits your plan sends after a claim. It shows what was billed, what the plan paid and what you may owe. It is not a bill.

Have both? Add both, and every page. The codes are often on the back or the last page.

A PDF, a photo or a scan. PDF, JPG, PNG or HEIC, up to 20 MB each, 25 MB in all, and 5 files.

By decoding, you agree to the Terms. We explain your letter; we do not give legal or medical advice.

No file? Type the denial reason or code yourselfYou get a shorter result: we can only explain what you type.

Short codes like 50, CO-50 or N115. On an EOB they are often in a "remarks" or "reason" column.

We need it to work out your deadline.

If you do not save the result, we delete what you type 24 hours after you send it.
