In short
- AppealHaven prepares documents and information. It is not a law firm, an insurer or a medical provider.
- You decide what to send, and you send it. We never act for you.
- We do not promise any outcome.
- You pay once per case. Refunds depend on the packet and our deadlines, never on the outcome.
- You can delete your data and close your account at any time.
1. Who we are
AppealHaven is run by [company legal name, address and state of incorporation: legal review]. In these terms, "we" and "us" mean that company, and "you" means the person using the service.
By using AppealHaven, you agree to these terms and to our Privacy Policy. If you do not agree, please do not use the service.
2. What the service does
AppealHaven is a document-preparation and information tool. It helps you understand a health insurance denial or a medical bill, and prepare documents you may choose to send. It includes:
- the denial decoder, which explains a denial letter or EOB (Explanation of Benefits) for free
- an appeal case: an appeal letter, request letters, an evidence checklist, deadline reminders and a PDF packet
- an optional bill audit: a comparison of an itemized bill with your EOB, and dispute letters
Paid cases are available for employer plans and for individual or marketplace plans. For Medicare, Medicare Advantage and Medicaid, we offer the free decoder and referrals to free help only.
3. What we are not
AppealHaven is not a law firm, an insurance company or a medical provider. We do not give legal advice or medical advice. Using AppealHaven does not create a lawyer-client or a doctor-patient relationship. AppealHaven is not a substitute for the advice of a lawyer.
We are not your authorized representative. We do not contact your health plan, your doctors, a hospital or anyone else on your behalf. Read our disclaimers for more.
4. You decide, and you send
You decide whether to appeal or dispute, what to say, and what to send. You can review and edit everything we prepare. You sign it, and you send it.
You are responsible for:
- checking that the details we read from your documents, and everything in our drafts, are correct and complete
- sending your appeal or request on time, in the way your plan or provider asks
- keeping copies of what you send, and a record of when you sent it
We work out deadlines from published federal rules and the date on your letter, and we show the rule we used. Your plan documents or state law can give you more time. If our date and your plan's date differ, confirm the date with your plan. [legal review: responsibility for deadline errors]
5. No promised outcome
Your health plan, or an independent reviewer, decides your appeal. A provider decides whether to change a bill. We cannot promise or predict those decisions. Nothing on AppealHaven is a promise that an appeal or dispute will succeed, or that you will save money.
6. Your account
- You do not need an account to use the decoder. You need one to start a case.
- You sign in with your email address and a 6-digit code we send to it. Anyone who can read your email can sign in, so keep your email account secure.
- Give accurate information, and keep your account to yourself. [legal review: using the service for a family member or as a caregiver]
- You must be at least 18 years old to create an account. [legal review]
7. Consent for health information
Before we use the health information in your documents for a case, we ask for your consent, separately from these terms. You can withdraw it at any time in your account settings. We then stop using that information and delete it as our Privacy Policy describes. Without that consent, we cannot prepare a case.
8. Prices and payment
- The denial decoder is free. An appeal case is $49. An appeal case with the bill audit is $79. Adding the bill audit to an appeal case later costs $30. All prices are one time, in US dollars.
- You pay by card through Stripe, our payment processor. We do not store your card number.
- There is no subscription, no automatic renewal, and no share of any money you save or recover.
- Prices may change for new cases. A change never affects a case you have already paid for.
- Taxes: [legal review: sales tax on digital services by state]
9. Refunds
If the packet we prepare is incomplete for your plan type, or a deadline we worked out is wrong, you can get a full refund. A refund never depends on what your plan or a provider decides.
[legal review: how to ask for a refund, the time limit, refunds for the $30 add-on, and state law requirements]
10. Acceptable use
You agree not to:
- upload documents about another person unless you have the right to act for them [legal review]
- use AppealHaven to make false or misleading statements to a health plan, a provider or a government agency
- resell the service, or use it to prepare documents for other people as a business
- try to break, overload, scrape or reverse engineer the service, or get into accounts that are not yours
- upload anything unlawful, or anything that infringes someone else's rights
11. Your documents
You own the documents you upload and the letters you create. You give us permission to store and process them only to provide the service to you, as our Privacy Policy describes. We do not sell them and we do not use them for advertising.
12. Outside information and links
We link to government sites, free-help programs and insurer pages. For UnitedHealthcare, Aetna and Cigna, we also show where to send an appeal, taken from their public pages. This information comes from sources we do not control and can change. If it differs from the instructions in your own letter, follow your letter.
13. Closing your account
- You can delete a case, or your whole account, at any time with one button. Deletion is permanent.
- We may suspend or close an account that breaks these terms, or when the law requires it. When we can, we will tell you why. [legal review: refunds on suspension]
- If we stop offering the service, we will tell you in advance and give you time to download your packets. [legal review: notice period]
14. Warranties and liability
[legal review] Warranty disclaimer, limitation of liability, any cap on damages, and the exceptions state law requires. To be written by counsel.
15. Disputes and governing law
[legal review] Governing law, where disputes are heard, an informal first step before any claim, and whether arbitration applies. To be written by counsel.
16. Changes to these terms
If we change these terms, we will update the date at the top. For important changes, we will email you before they take effect. [legal review: notice period]
